This axial (cross sectional) contrast enhanced CT through the upper part of the chest and mediastinum (the central part of the chest) shows a rounded density with central lower density which represents a necrotic, metastatic (malignant) lymph node from a cancer outside of the chest. This indicates spread of the cancer to the lymph nodes in the chest. Other diseases such as tuberculosis or fungal diseases can also result in necrotic lymph nodes in the chest.
