At upper right, a sagittal view shows the intestines protruding through the abdominal wall via an old surgical incision. At upper centre, the hernia is seen from the front. At upper right, the old incision is re-opened. At lower left, with retractors holding back the overlying tissues, the omental attachments (part of the layer of peritoneum surrounding the internal organs) are cut away in circumferential fashion to form a circular hole. At lower centre, a reinforcing mesh is put in place, sutured to the fascia. At lower right, the incision is re-closed.
