This axial (cross sectional) CT image of a newborn shows replacement of most of the cerebral hemispheres with fluid density. There is some residual brain parenchyma posteriorally, towards the bottom of the image. This residual parenchyma consists of the thalami and posterior fossa which is typical in cases of hydranencephaly. This is the typical appearance of hydranencephaly which is essentially destructive changes involving the cerebral hemispheres related to an in-uteru insult. There is often residual posterior fossa parenchyma seen as in this case. The differential diagnosis includes holoprosencephaly and marked hydrocephalus.
