This sagittal (from the side) T2 weighted image of the lumbar spine shows the typical appearance of a large intradural lipoma with a tethered spinal cord. Normally the spinal cord does not terminate below the L2-3 level. In this case the large lipoma within the sacral spinal canal is preventing the normal ascent of the cord during development. This results in an abnormally low termination of the spinal cord which can result in neurologic disability.
