Bruised toenail of a 15 year old girl being drained by a nurse. The blood had collected under the nail of the big toe, a condition known as a subungual haematoma, which was caused by blood vessels beneath the nail rupturing. In most cases, the blood drains away by itself after a few days. However some cases, such as this, require surgical drainage (trephination) by lancing a hole in the nail.
