This axial (cross sectional) CT image of the brain in a neonate shows the typical appearance of hydranencephaly. This is the result of a severe in utero insult to the brain resulting in essentially necrosis/destruction of the cerebral hemispheres. Often the posterior fossa contents, brainstem and thalami are preserved. The differential diagnosis is severe hydrocephalus. In hydranencephaly the head size is small. In hydrocephalus the head size is abnormally large for age.
