Torn perineum. Tear (lower centre) in the perineum of a 39-year-old woman three days after childbirth. The perineum is the tissue between the vagina and the anus, which may tear during childbirth. An episiotomy procedure, where a surgical incision is made in the perineum, may be performed before the baby is born. This enlarges the vaginal opening, which eases the delivery of the baby and prevents uncontrolled tearing.
