Child with kwashiorkor. Kwashiorkor is a form of severe malnutrition characterised by oedema (fluid accumulation) leading to swelling of the belly (as in this case). It also causes an enlarged fatty liver (as here), hairloss, loss of teeth and dermatitis. It is common in areas of famine.
